How to Collect Your ProxiGene Sample

Your ProxiGene sample collection is quick and easy. Once your kit arrives, follow the seven steps below.

  1. Step 1

    Moisten the swab

    Place the swab on your tongue, close your mouth, and allow it to moisten with saliva for 10 seconds.

  2. Step 2

    Swab BOTH cheeks

    Place the swab between your cheek and gum. Press lightly on the outside of your cheek and rub the swab firmly back and forth 10 times. Repeat 10 times on the other cheek.

  3. Step 3

    Close the tube

    Place the swab inside the sample tube and close the cap as tightly as possible, until it cannot be tightened any further.

  4. Step 4

    Label the tube

    Apply the provided barcode label from your patient form to the sample tube. Make sure the barcode is clearly visible.

  5. Step 5

    Bag the sample

    Place the labeled sample tube inside the provided biohazard bag and seal it securely.

  6. Step 6

    Complete and SIGN the Patient Form

    Fill in the Patient Form (requisition form) using your own information and following the instructions supplied with your kit. Please write as clearly and neatly as possible, illegible handwriting can delay or affect how your results are sent. Complete and SIGN the consent and authorization section. Your results will be sent directly to you.

  7. Step 7

    Pack Your Kit

    Insert your completed patient form and place it, along with the sealed biohazard bag, inside the kit box. Close the box and secure the flap with the provided adhesive dot. Record the tracking number from your return shipping label so you can follow your sample's delivery back to the lab.

Final check

Before Returning Your Kit

  • Sample tube is completely closed
  • Barcode label is attached and visible
  • Biohazard bag is securely sealed
  • Patient form is completed and signed
  • Patient form is inside the kit box
  • Return tracking number is recorded
